A company is looking for an RN Cardiac Health Coach -
Wellness.
Key Responsibilities
Complete health assessments and implement care plans for members
with cardiac conditions
Provide telephonic support and coaching to improve members' health
and lifestyle
Monitor wellness plans and adjust care recommendations based on
member needs
Required Qualifications
Associate's degree with 2 - 4 years of related experience
Registered Nurse (RN) license required
Health Coach Certification or equivalent certification in a related
field
Experience with chronic health conditions, particularly cardiac
issues
Ability to engage and influence members in health-related
decisions
