  • Must Be/Have: Experienced CM --- a RN --- Discharge planning assessment exp --- Excellent UR --- Psych social assessment skills --- Acute hospital setting experience. Requires doing InterQuals & screening transfers for clinical appropriateness.
  • **BSN preferred * MUST HAVE EPIC EXP.
